CANCELLATION POLICY

Appointment & Cancellation Policy BODYBYYASSY TERMS, CONDITIONS, CONSENT, AND NO REFUND POLICY BY CONFIRMING THIS APPOINTMENT, I acknowledge that I have carefully read and fully understand the contents of this agreement. I have had the opportunity to ask questions regarding the treatment and all of my questions have been answered to my satisfaction. ASSUMPTION OF RISK I desire to participate in certain aesthetic, spa, and/or beauty services offered by BodyByYassy ("Business"). I understand that participation in these services involves inherent risks, including but not limited to discomfort, bruising, swelling, infection, allergic reaction, unsatisfactory results, temporary or permanent injury, emotional distress, financial loss, disability, or, in rare circumstances, serious complications. I understand that no guarantees or warranties have been made regarding the results of any treatment and that individual outcomes vary. I acknowledge that BodyByYassy cannot guarantee any physical, cosmetic, or psychological benefits from services rendered. WITH KNOWLEDGE OF THESE RISKS, I VOLUNTARILY REQUEST SERVICES AND AGREE TO ACCEPT AND ASSUME ALL RISKS OF ILLNESS, PERSONAL INJURY, PAIN, SUFFERING, TEMPORARY OR PERMANENT DISABILITY, DEATH, PROPERTY DAMAGE, OR FINANCIAL LOSS ARISING FROM SUCH SERVICES, WHETHER CAUSED BY ORDINARY NEGLIGENCE OR OTHERWISE. RELEASE OF LIABILITY I hereby voluntarily release, waive, discharge, and hold harmless BodyByYassy, its owners, employees, independent contractors, medical professionals, agents, successors, and assigns from any and all claims, demands, causes of action, damages, liabilities, costs, and expenses arising out of or related to my presence on the premises and receipt of spa, beauty, wellness, or aesthetic services, except where prohibited by applicable law. MEDICAL DISCLOSURE I certify that I have truthfully disclosed all medical conditions, allergies, medications, supplements, and physical limitations. I understand that withholding information or providing false information may increase the risk of complications and may affect treatment outcomes. I agree to notify my provider immediately of any changes to my health status before future appointments. PAYMENT POLICY Payment is due at the time services are rendered. BodyByYassy accepts cash, debit cards, credit cards, Affirm, Afterpay, Klarna, and Pay with Cherry financing. NO REFUND POLICY ALL SALES ARE FINAL. Due to the nature of aesthetic, spa, beauty, and wellness services, NO REFUNDS WILL BE ISSUED FOR ANY REASON, INCLUDING BUT NOT LIMITED TO: • Cash payments • Debit card payments • Credit card payments • Affirm financing • Afterpay financing • Klarna financing • Pay with Cherry financing By proceeding with treatment, I acknowledge that I understand and accept that all services and products are final sale and non-refundable. Results are not guaranteed and additional treatments may be required to achieve desired outcomes. APPOINTMENT POLICY Deposits are non-refundable. Same-day cancellations may be subject to an 85% cancellation fee. No-call/no-show appointments may be charged 100% of the scheduled service amount. IDENTIFICATION REQUIREMENT For the safety and protection of all clients and staff, a valid government-issued photo ID may be required before treatment. Failure to provide identification may result in cancellation of the appointment. ENTIRE AGREEMENT This Agreement constitutes the entire understanding between BodyByYassy and the client and supersedes all prior discussions, representations, and agreements. If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect. This Agreement shall be governed by and interpreted under the laws of the State of New York. BY CONFIRMING THIS APPOINTMENT, CHECKING THE CONSENT BOX, ELECTRONICALLY SIGNING, OR RECEIVING SERVICES, I ACKNOWLEDGE THAT I HAVE READ, UNDERSTAND, AND VOLUNTARILY AGREE TO ALL TERMS, CONDITIONS, POLICIES, RELEASES, AND CONSENTS CONTAINED HEREIN.
